Fortunately, there are many effective infertility treatments available today for both men and women. Treatment outcomes have improved over the last several years, and it is estimated that approximately 85% of patients who seek in fertility specialist care will achieve pregnancy.
It is extremely important that a fertility specialist be consulted early, especially if the wife is 35 years or older. A specialist will order complete fertility work-ups for both the husband and wife to insure that all contributing factors are identified. Specialists are also much more likely to move couples rapidly through available, appropriate, treatment options. Sometimes we see women who have been on multiple cycles of Clomid when clearly it is not likely to work after three ovulatory cycles.

Some degree of male infertility is present in up to half of all infertile couples. This makes the semen analysis a critical part of the fertility evaluation. No female treatment should begin until male infertility has been ruled out.
Fortunately, most couples under a specialist’s care will become pregnant without the need for in vitro fertilization (IVF). Studies suggest that less and 15% of all couples that see a fertility specialist will require IVF. Of course this percentage varies dependent upon many factors such as the geographical location of a physician’s office and the nature of the practice.
Treatments can be divided into three categories including medical, surgical and assisted reproductive technologies which includes IVF and the donor egg program. Medical treatments include medications such as Clomid, follicle stimulating hormone, human chorionic gonadotropin (hCG), progesterone, Lupron, and many others. Most medical treatments are aimed at achieving regular ovulation. Many times treatments such as ovarian stimulation with FSH are combined with techniques such as intrauterine insemination (IUI). In an IUI cycle, the ovaries are stimulated with medication, and the husband’s sperm is washed and concentrated and placed directly in the uterus. Couples can also opt to use sperm from a donor.
Surgical treatments include removal of endometriosis, correction of uterine abnormalities, removal of polyps or fibroids, or repairing congenital disorders. Surgery may also be employed to reverse tubal sterilization by reconnecting the fallopian tubes. In the male, surgery may be employed to correct a varicocele, remove blockages, or reverse a vasectomy.
Assisted reproductive technologies include a myriad
of treatments, many of which have come into routine clinical usage
over the last 15 years. In
vitro fertilization is commonly employed in many conditions,
including moderate to severe male factor, older women, those who
fail other treatments, in cases of tubal blockage or damage, and
many other conditions. Our donor egg program gives new hope to many women who cannot use their
own eggs. Our IVF success rates are among the highest in the country.
Micromanipulation procedures used in conjunction with in vitro fertilization have allowed thousands of otherwise infertile couples to create children. Men who previously had no chance to produce a genetically related child can now father children using an IVF laboratory procedure known as intracytoplasmic sperm injection (ICSI). Using ICSI, a single sperm is directed directly into the egg. The sperm can be obtained from the ejaculate or the reproductive tract (testicular biopsy). Blastocyst transfer has increased pregnancy rates, while lowering the incidence of multiple births. Additionally, preimplantation genetic diagnosis PGD (performed in our Dallas Texas Clinic) provides a means to effectively screen and prevent several genetic diseases in offspring.
One of the best means to control infertility treatment costs is to see a fertility specialist early in the evaluation. It is much more likely that the appropriate treatment will be rendered early in the course of therapy.
